Initial Health Intake
& Acupuncture Treatment
Harmonizing Acupuncture Treatment
(10 pre-selected points to alleviate stress, regulate immunity, and support digestion - no individualized diagnosis provided)
ADD-ONS to ACUPUNCTURE
Tuina (12-15 minutes of Chinese medical massage)
In order to maximize the benefits of your treatment, we suggest arriving 10-15 minutes early, so you can begin relaxing. Plan on spending 75-90 minutes for your initial office visit, to allow time for a detailed health history, physical exam, and acupuncture treatment. Return visits are approximately 50-60 minutes.
It is helpful to wear comfortable, loose fitting clothing that can be rolled up to the knees and elbows, but gowns are available if needed. Additionally, try to eat a light snack a couple of hours prior to treatment so you do not experience any light-headedness. Do not brush or scrape your tongue as this can disrupt the natural coat which is inspected during examination, and please refrain from using caffeine, alcohol or other drugs prior to treatment. We also encourage you to avoid rigorous physical activity two hours before and after your treatment. As a courtesy to our other patients, please keep your voices low, turn cell phones off or to vibrate, and avoid wearing strong perfumes.
Cancellation & Punctuality Policy
Our office has a 24-hour cancellation policy. If you need to cancel or reschedule, kindly contact us at least 24 hours prior to your appointment to avoid being charged. If you are unable to cancel within 24 hours, you will be held responsible for your visit at a minimum payment of $75. It is our priority to spend quality time with each patient, therefore we are unable to accommodate patients who are more than 15 minutes late, as this will affect other patients. If you are late, we may not be able to treat you, and you will be held responsible for the cost of your visit.
Payment & Insurance
Fees for services and products are due and payable in full at the time of your appointment. We accept credit card, checks and cash.
We are in network with several insurance companies, so please contact us to inquire about your health plan and acupuncture benefits. Insurance billing and submission of medical insurance claims will be handled by our office as a professional courtesy, and payment of co-pays, co-insurance, and deductibles are expected at the time of service.
For health plans that do not have out-of-network coverage, we can provide you with a completed superbill upon request. This is a universal form that provides all of the information and codes needed for insurance reimbursement. Simply submit this form to your insurance company along with your policy number, and reimbursement will be sent directly to you.
